## Before combining issues

1. Can another researcher locate the same source segment from the ID?
2. Are action, interpretation, and recommendation separate? Is unobserved information still marked unknown?
3. Do combined issues share a context, or merely a label?
4. If a change has been implemented, where is the follow-up check recorded? A recommendation is not an outcome.
